Deliver care to clients in accordance with the state Nurse Practice Act, agency policies, and recognized professional standards. The Registered Nurse utilizes a broad foundation of nursing theory and processes to provide care while accurately documenting and communicating observations and assessments.

Report to: Client Services Director/Nursing Supervisor QUALIFICATIONS:

• Graduate of an accredited school of professional nursing; BSN or PHN preferred
• Holds a valid Registered Nurse license in the state(s) of practice

JOB DESCRIPTION: RN CASE MANAGER POSITION SUMMARY:

Manage a group of clients by applying nursing theory and processes, following the guidelines of the state Nurse Practice Act.

Report to: Director of Client Services (Director of Nursing) Direct: Assigned team members of RNs, LPNs, Home Health Aides and Personal Care Assistants QUALIFICATIONS:

• A graduate from a recognized and accredited professional nursing school
• Current license as a Registered Nurse in the state(s) of practice; a baccalaureate degree is preferred

JOB DESCRIPTION: RECEPTIONIST POSITION SUMMARY:

Greet visitors and receive incoming telephone calls and fax transmissions. Transfer calls appropriately. Provide clerical support and assist with projects as delegated.

QUALIFICATIONS:

• At least two years of nursing experience in an acute hospital setting or a similar role, with one year in a supervisory position
• Strong verbal and written communication abilities, along with excellent interpersonal skills

• Understands reimbursement methods and documentation requirements in the home healthcare industry

The Homemaker/Companion works under the supervision of a Registered Nurse and performs tasks that contribute to clients’ comfort and safety in the home. The Homemaker/Companion offers emotional support, social interaction, quiet companionship, and assistance with meal preparation and light housekeeping tasks.

JOB DESCRIPTION: HOME HEALTH AIDE POSITION SUMMARY:

Deliver personal care services under the supervision of a Registered Nurse or Therapist. Assigned to specific clients by the Registered Nurse or other qualified professional, the Home Health Aide provides services to ensure the client’s personal comfort.

Report to: Client Services Director/Nursing Supervisor QUALIFICATIONS:

• Completion of an approved certification training program, written skills test, and competency evaluation
• At least eighteen (18) years of age
• At least six (6) months of work experience in a supervised setting, preferably in a healthcare facility
• Proven ability to read, write, and follow a written Plan of Care
• Good interpersonal skills
• Current valid driver’s license, good driving record, and reliable transportation

JOB DESCRIPTION: HOME HEALTH AIDE POSITION SUMMARY:

The Home Health Aide operates under the supervision of a Registered Nurse or therapist, providing services to ensure the client’s personal comfort is maintained.

Report to: Nursing Supervisor QUALIFICATIONS:

• Must have completed one of the following:
  • Completion of a 75-hour training course and competency assessment, or
  • A competency evaluation approved by the Commissioner or
  • Equivalent training in another jurisdiction that matches the 75-hour course or
  • Participation in a training or competency evaluation program that meets Medicare requirements for Home Health Aides, as outlined in Title 42, Section 484.36 of the Code of Federal Regulations.
• Have satisfied the following requirements:
  • Fulfill the requirements outlined in Title XVIII of the Social Security Act for Nursing Assistants in Medicare-certified nursing facilities or successfully complete a Nursing Assistant training program approved by the State of Minnesota; and
  • Have had at least twenty (20) hours of supervised practical training or experience performing Home Health Aide tasks in a home setting under the supervision of a Registered Nurse or finish the supervised practical training or gain relevant experience within one (1) month of starting work on Home Health Aide duties; or
  • Completed an approved Home Health Aide training program before June 1, 1993.
• Be at least eighteen (18) years of age.

• Be proficient in speaking, reading, and writing English to follow the Plan of Care and complete the required documentation.
• Demonstrate maturity to handle the responsibilities of the role effectively.

• Maintain good physical and mental health, including up-to-date TB testing.
